  1. I am a registered GP at Wyre Forest Health Partnership so cannot see any patients privately that are registered with Church Street Surgery, Hagley Surgery  or  Stourport Medical Centre , Kidderminster Medical Center, Bewdley Medical Center.

  3. Choose the date and time you would like.
  4. Fill in your name, email, phone number.
  5. Give a short descriptive reason for your appointment in the extra info section.
  6. Then please make your payment.
